Information you send with the form

The form asks for your name and phone number, and lets you add an email address, a ZIP code, the kind of injury and a few words about what happened. Please do not send medical records, Social Security numbers or other sensitive documents through the form.

Who receives your request

Participating attorneys are independent. Once an attorney’s office has your information, its own privacy practices apply to how it uses it.

Calling the referral line

The phone number on this site is a tracking number. When you call it, the phone service records details such as your phone number and the time and length of the call, and connects you with a participating attorney’s office. Calls may be recorded for quality and training purposes. By continuing a call after the recording notice, you agree to the recording.

Texts and automated calls

The operator and participating attorneys or their intake representatives contact you by phone call or text message, including by automated technology, only if you check the consent box on the form. Consent is not a condition of getting a referral. Message and data rates may apply. You can reply STOP to any text to opt out, or email the operator.

Cookies and tracking technology

What you type in the form is not sent to advertising or analytics services. Like most websites, the site’s hosting and security services process technical information such as your IP address and browser type to deliver pages and protect the site from abuse, and may set cookies needed for that. If the site adds analytics or advertising tags, this policy will be updated to say so.

How long information is kept

Requests and consent records are kept as long as needed to respond, to show that consent was given, and to meet legal duties, and then deleted.
